New Patients
If you are a new patient and would like to schedule an appointment with a Primary Care Proivder or one of our Specialists, please call our Clinic Operator at (805) 681-7500. Our appointment schedulers will be able to match you with a physician or other provider at the location where you will receive your care.
If you are a new patient and would like to schedule an appointment at one of our Distinguished Specialty Centers, please call that center directly. For more information and links to the phone numbers for our Distinguished Specialty Centers click here.
New patients with a Physician Referral, please call our Clinic Operator at (805) 681-7500. Our appointment schedulers will be able to connect you to the department or specialist to which you are being referred.
Existing Patients

MyChart Activated Patients
