The Anticoagulation Clinic (ACC) (phone, fax, hours, directions) is a specialized clinic that focuses predominantly on the management of anticoagulation with Coumadin. The mission of the ACC is to provide safe and effective anticoagulation therapy.

Anticoagulation Services

The ACC providers, including Nurse Practitioners (NPs) and Physician Assistants (PAs), who have had specialized training in Anticoagulation Therapy Management, coordinate and optimize anticoagulation therapy by:

• Managing anticoagulation dosing
• Providing systematic monitoring and patient evaluation
• Providing ongoing education, including monthly coumadin education classes open to the public

• Communicating with other providers involved in the patient's care

Conditions we commonly treat include a trial fibrillation, mechanical heart valves, deep vein thrombosis, pulmonary embolism, CVA, TIA and other conditions that require anticoagulation therapy with coumadin/warfarin.
